A new study in the Journal of Clinical Psychiatry suggests that antidepressants may be less effective at treating attention-deficit/hyperactivity disorder (ADHD) compared to placebo. In the latest paper, from the University of California, San Francisco (UCSF), conducted by the Medical Director of the Department of Psychiatry, and colleagues, the authors found that a combination of antidepressants, including (SSRIs) and (TCAs), was superior to a placebo at reducing both ADHD symptoms and overall functioning in adults with ADHD.
In children with ADHD who were treated with a combination of (SSRIs), only 25 percent of children reported a noticeable improvement, compared to only 38 percent of those treated with a placebo. In children who had ADHD, the mean ADHD symptom score decreased from 0.6 to 0.3 in children treated with SSRIs compared to a mean change of -2.3 points of -2.5 points in those who received placebo. In addition, a higher percentage of children with ADHD treated with a combination of (TCAs) reported a reduction in their ADHD symptoms compared to those treated with a placebo.
Researchers from the UCSF School of Medicine and the Urology and Neurology Department at UCSF enrolled 2,907 children and adolescents with ADHD. The researchers followed these children over the course of 12 months and found that a combination of SSRIs and (TCAs) resulted in a statistically significant reduction in ADHD symptoms. The effect was maintained over the same period, suggesting that a combination of antidepressants may be more effective at improving symptoms in children treated with SSRIs than the use of a placebo.
“The combination of antidepressants (SSRIs and TCAs) has a good safety profile in children, and the findings in this study add to the growing body of evidence that SSRIs may be more effective at improving ADHD symptoms than TCAs,” said co-author Dr. James A. Cahn, M. D., associate professor of psychiatry and child and adolescent psychiatry and director of the UCSF School of Medicine’s Child Health Program. “This is the first study to show that combining SSRIs and (TCAs) is equally effective at improving ADHD symptoms, in terms of improving ADHD symptoms and overall functioning,” Dr. Cahn added.
The results were published online in the Journal of Clinical Psychiatry. The study involved 2,907 children with ADHD. The investigators randomly assigned the children to one of the two SSRIs (sertraline, fluoxetine, paroxetine, fluvoxamine, norfloxacin, sertraline and venlafaxine) or one of the TCAs (fluoxetine, paroxetine, fluvoxamine, sertraline and sertraline).
Sertraline was chosen because it was the most commonly prescribed medication in the pediatric population, and the data suggest that it may be a more effective alternative to (TCAs) for children with ADHD.
Sertraline was administered to 4,071 children. Of those, 897 had ADHD, and 897 children had ADHD in either the SSRIs group or the TCAs group. Of those, 796 children had ADHD in either group.
